The Sober Transitions program at Turning Point Centers is designed to help the client slowly, safely transition back into life after rehab. The first year of sobriety can be difficult. Clients and their families are faced with putting to work the tools they’ve learned in treatment in the real world. The fear and uncertainty associated with moving forward is one that typically causes anxiety for the client and their family. This program addresses those concerns, fears and gives encouragement, education and counseling through the stressful times. Very often clients become anxious and nervous when leaving the safe, structured environment of our rehab program. This commonly not only happens to the client…it happens to the family and friends as well. Taking that burden of stress off the client’s shoulders gives them a greater chance to maintain sobriety.
With the help of the counselor a client can learn the necessary steps to remain sober the first year out of treatment. The counselor will also help with the everyday challenges that will arise with the new sobriety. These challenges can be anything from getting a new job, trying to figure out what to do with life, anger, goal setting, re-gaining and re-building relationships, relapse all the way to the simplicity of maintaining a schedule and dealing with simple triggers. Sometimes just reintegrating back into the home environment can be a stressful time.

Phase 1: Months 1 - 3
This phase is the most intensive of the 4 phases. Communication, drug and alcohol testing and counseling is at the highest level of the program. The client will meet with the counselor to set out goals and discuss the future and his/her expectations. From this evaluation the counselor will be able to set up the individual plan. Each client will follow the schedule listed below:
Phase 2: Months 4 - 6
Phase 2 is very similar but again, “steps” the client into more responsibility and freedom. Each client will follow the schedule below:
Phase 3: Months 7 – 11
By this point in the program the client should truly be starting to gain comfort in his/her sober lifestyle and activities. Each client will follow the schedule below:
Phase 4: Month 12
The final phase of the program is the Relapse Prevention phase. It’s in this phase that the client and the counselor discuss the program, what’s been learned, the accomplishments and celebrations and the direction for maintaining sobriety. During this phase the client and counselor meet weekly to evaluate the relapse prevention plan. There is also one final, random, drug and alcohol test.
After completion of this program the client can continue to come to the Aftercare program and is encouraged to stay in touch with Turning Point Centers and the staff!
